Introductory and guiding texts on the survey form

Currently the survey form only presents the fields to be filled in and no guidance except for the pretty hidden field descriptions.

The field group module can be used to add text parts and other elements to the content type forms that are used in the survey.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
